Don't roll if the character has been dead less than one minute.

Roll 1d100. If a result doesn't make sense or doesn't stack with an existing effect, reroll. If you roll a 00 on the dice, roll again twice.

  • Add 10 for each time you were resurrected before this.
  • Add 5 if you were dead for more than a day, plus 5 more if you were dead for more than a week, 5 more if more than a month, and 5 for each year you've been dead.
  • Add 10 for each age category reached, starting with middle age.
  • Subtract 5 if you were dead for less than an hour.
  • Subtract 10 if you were raised with resurrection. Subtract 25 if you were raised with true resurrection. Subtract 50 if you were raised with reincarnation.
  • Subtract 15 if you were raised during your deity's month. Subtract 15 if you were raised on a relevant holy day.
  • Special preparations, rituals or sacrifices can reduce this result further. Inappropriate details could increase it.

If you have not picked a single deity and the results mention one, choose one of those you revere randomly. If you don't revere any deity in particular, pick one randomly from those within one step of your alignment. If doing this doesn't make sense, reroll the result.

In most cases, you will suffer some penalties to initial reaction checks and diplomacy checks with people who notice your condition. It is possible to rid yourself of any of these effects given enough time and effort.

1 ...-15 No side effects. Consider yourself very lucky.
2 16-19 The strain of death and resurrection has left you sickly, weak or confused. Take enough ability damage to reduce a random stat to 1. This particular source of Con damage cannot reduce your maximum hit points below 1.
3 20-28 Death marks show up on your body, but fortunately they're easy to hide or fairly subtle.
4 29-30 Reality feels more like a dream often leaving you dazed and confused especially in stressful situations. You are dazed for one round when surprised (such as during your first round in a combat you didn't initiate). This wears off after a week.
5 31-34 Fragments of old souls give you visions of the past. Occasionally, you get a deja vu feeling from people or places you've never seen.
6 35-42 You've developed a fear or hatred towards whatever killed you (you decide). You take a -4 on actions counter to your choice (such as fleeing from a hatred or attacking a fear). This penalty is reduced by 1 after each such encounter.
7 43-49 While you were dead, your deity showed you something important. Now that you're back, you can't remember all the details though.
8 50-52 The ordeal has drained the color from your hair and skin leaving you an albino.
9 53-64 Death marks show up on your body. Superstitious people will react badly to you and even non-superstitious people might be a bit wary.
10 65-67 The scent of death clings to you though only enough for animals (or those with the scent ability) to detect.
11 68-75 Your deity has marked you in a visible way. People can see who has claimed your soul, and even if you switch religions, you're already theirs.
12 76-79 You still can still see the afterlife. Most of the time you can tell what's real and what isn't, but sometimes you get the two slightly confused.
13 80-82 A weakened link between your soul and body leaves you vulnerable to possession, especially while you sleep.
14 83-88 Another ghost followed you back and is now haunting you. Only you can see and hear them, but they won't leave you alone.
15 89-96 Your connection to your body is severely weakened. Next time, you'll have to settle for reincarnation or true resurrection.
16 97-101 Your body's connection to the elemental planes is thrown out of balance. Choose one of the four elements randomly. This can manifest in various ways, but usually causes both a physical symptom and affects the character's humors. (Look at the elemental-touched races if you need some ideas.) If you don't feel up to roleplaying this, you can reroll this result.
17 102-108 You want to return, and your deity is allowing it, but you can't find your way back to your body. Your friends must enter the afterlife in a dream-state to guide you back.
18 109-115 You made it back as far as the ethereal plane, but something stopped you there. You need to find a way to get to the material plane and to your body.
19 116-118 You've returned, but someone else got to your body first. Now you're haunting them at least until your friends notice. If you don't feel up to roleplaying this, you can reroll this result.
20 119-123 Your ordeal has left you with amnesia. You don't remember anything from before you were resurrected, although you skills and abilities remain intact. This will fade over 1d4 weeks.
21 124-125 Your time in the afterlife has given you a new perspective on this life. Move your alignment one step in a random direction. If you don't feel up to roleplaying this change, you can reroll this result.
22 126-130 The spell went slightly awry and now your soul is partially bound to some object present at your resurrection, most likely something already on your body. If you're more than 100 feet away from this object, you're sickened until you get it back. If the object is destroyed you are knocked unconscious for 1 minute and gain two permanent negative levels.
23 131-137 Your deity sent you back, with conditions. You are geased to complete this quest before your normal life can resume.
24 138-140 Something malevolent came back with you. Roll Init.
25 141-143 Your connection to the positive energy plane was weakened leaving your body unable to heal naturally. Magically healing still works normally though it is likely to leave unsightly scars. You still roll normally to staunch bleeding.
26 144-147 Something went wrong and now your body is connected to the negative energy plane instead of the positive energy plane (gain Negative Energy Affinity). Fixing this will require some effort. (Reverse this if you already had Negative Energy Affinity.)
27 148-150 Your deity has decided that enough is enough and won't let your soul go for just a diamond.
28 151-... Your ties to the afterlife are too strong to resurrect you fully. You come back still tied to the outer planes. Your type changes to native outsider though none of your stats change from this. If you roll this a second time, you begin to take on physical traits of your afterlife self.

If you want to pick randomly, roll on the following table to determine where death marks show up. If the result doesn't make sense, reroll.
