THis Strange is not the sorcerer supreme in 1888 - not yet. Also the reason he does do surgery any more wasn't an accident - it's a choice.

When the Avengers see him, it would have been the 1st time he had been back in England in years. I really think he needs to have a public identity therefore I am saying he's been traveling the word, studying the mystic arts.
Dr. Estrange is no longer a member of the Masons for he has indeed uncovered a secret sect within, lead by Lord Mordo. Estrange trusted him, his medical teachings & certain 'advanced surgical procedure' which Estrange took part in. Steven discovered that he wasn't helping people but making slaves for Mordo's sinister cult, which Estrange only knows that they worship some being called "Dormammu". Fury maybe the one who helped him learn the truth about Mordo.
It was too much to bear, he quit his post at the hospital, & traveled to the Far East to learn the ways of the Light, to return to England & combat the Dark & atone for the pain he had caused. Having him practice medicine in any way is a Complication for him. Also I was considering having him have an addiction to Opium but I'm not sure about that one.
