The best "all italian" game ever published is IMHO Lex Arcana: the first and sadly last attempt made by a big italian game publisher (DalNegro, traditionally a game-card publisher) to create from nothing a new game. The setting is wondrous, the mechanics original and very appealing, and the character creation and progression system truly genious-like. Alas, the dream lasted just enough to make the game distributed in Italy, and then died out (Nexus game tried to relieve the game line, but they also quitted after a short time).
