Melee combat: Attacker gains bonuses to their to-hit. Both combatants exchange hits

If player takes a certain large amount of damage in one attack, they must make a dismember check. If they fail,

they must roll for lost limb (very small chance of losing the head). Two kinds of dismembering, sever or vaporize.

The latter does not allow for the limb to be recovered.
